How many forms do I need to mail in?

For the 50th reunion, the only form you need to mail in is the REGISTRATION FORM (see next heading). This takes care of all costs except for hotel room reservation. To reserve your hotel room, you don't need to fill out any form - you can do that on-line or by phone.  Click on "Links" in the top-of-page menu, then under "Lodging and Main Event Location", click on "Reunion Hotel", and then follow instructions there.

WARNING:  DO NOT PUT OFF RESERVING YOUR ROOM!  You need to get your room NOW!  IF you make your room reservations early, our reunion hotel is plenty big enough for us.  It has 301 guest rooms.  BUT . . . if you wait until the last minute, you're liable to find yourself out-of-luck, and commuting in to the reunion hotel festivities from some remote hotel that's not as nice, and more expensive.  For future events, any required forms will be posted here. Also, if there later develops an initiative to put together a bio information booklet, a form will be posted here.

The link below opens a window that displays the Registration Form for our 50th reunion, in Acrobat Reader format (PDF)  NOTE: The form below is the latest incarnation.  Use this, and NOT the form from the back of the Feb 2010 Newsletter.  This most up-to-date form (July 2010) collects your meal selection (beef, chicken, fish) and miscellaneous additional information.  If you already sent in the old form, you don't need to refile.  Look for instructions in the forthcoming July newsletter.  It will provide instructions as to what supplemental info you need to send in via email.

If your computer doesn't have Acrobat Reader (most do), click on the link below, just above "Let's take it from the top".

Note:  To print out the registration form, click on the link below to bring up the form.  Then click on the printer icon in the top toolbar of the Acrobat Reader window, and then hit "Enter" on your keyboard.  You can't send in your registration via this web site.  Print out the form, fill it out manually, and mail it in along with your check.
